“Pursuant to Article 13 Sections 1 and 2 of the Regulation (EC) No. 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (OJ L 119, p. 1), hereinafter referred to as “GDPR,” in connection with your inquiry, we wish to inform you that:

1. The controller of your personal data is MOKATE S.A., based in Ustroń (43-450) at ul. Katowicka 265A, entered in the Register of Entrepreneurs of Krajowy Rejestr Sądowy (KRS, National Court Register) under the KRS number 0000010037;

3. Your personal data will be processed for the purposes related to submitting an inquiry via the contact form – based on your consent, which is compliant with the provisions of Article 6 Section 1a of the GDPR;
4. The recipient of your personal data are companies in the MOKATE Group – MOKATE Sp. z o.o., based in Żory, Global Coffee Group Sp. z o.o., based in Ustroń, and FPUH MOKATE Sp. z o.o., based in Ustroń;
5. Your personal data will be stored for the validity period of claims in connection with the handling of submitted inquiry;
6. You have the right to access your data and to rectify and delete data, restrict data processing, the right to data portability, the right to raise objections, the right to withdraw consent to data processing at any time without affecting the lawfulness of processing based on consent given before its withdrawal;
7. All communication with regard to your data, including a statement on withdrawal of consent to the processing of personal data, should be submitted to the Data Protection Officer indicated in point 2), in writing or by e-mail (to the addresses given in point 2));
8. You have the right to lodge a complaint with the President of the Personal Data Protection Office, if you deem that the processing of your personal data violates the provisions of GDPR;
9. The provision of your personal data is voluntary, but necessary for the purposes of handling your inquiry.”
